Our mission is to help families navigate early-intervention services with dignity, respect, and professionalism. Under the leadership of Founder, Owner, and Operator Chardell Young, each young child receives personalized attention designed to support development, participation, and family confidence.

What does a case manager do?+
A case manager listens to your needs, helps develop goals, coordinates services, connects you with community resources, and supports communication among the people involved in your care or support plan.
What ages does Katch Me Up serve?+
Our program serves children from birth through age three and their families. Service eligibility and availability are confirmed during intake.
Who may request services?+
Parents, guardians, caregivers, pediatric providers, EarlySteps partners, and professional referral sources may submit a request for a child from birth through age three.
What areas do you serve?+
Katch Me Up Case Management proudly serves Louisiana Region 8. Contact the office to confirm service availability for your specific location.
What happens after I submit a request?+
Your request is sent directly to Founder, Owner, and Operator Chardell Young. The office will review the information and follow up using the phone number or email address you provide.
Should I include medical records in the form?+
No. The website form is only for an initial service request. Do not submit medical records, Social Security numbers, insurance numbers, or emergency information through this form.
